To prepare this segment, we pulled together best practices from SDOH intervention programs around the country: resource directories, referral networks, community information exchanges - and extracted seven essential components.

Social Determinants of Health is a concept on everyone’s lips these days, but there’s also a wide variety of opinions and perspectives on what it all means ... and what to do about it. We think that SDOH is the future, and the more clarity and consistency we can bring to the space the faster it can go from a hashtag to a discipline. The SDOH Field Guide tries to answer SDOH questions in a pragmatic, no-nonsense way.
