WHO WE ARE

Specialists in Community Collaboration

Advocate Advisors was founded with a simple mission: expand the powerful potential of social collaboration networks, especially around social determinants of health (SDOH).

You may call them community information exchangesreferral networks, or even simply a resource directory, but they all point to the same vision: when community, municipal and healthcare organizations team up, they can achieve better outcomes, save money, and make peoples' lives better.

Our team has nationwide experience in the development of these cross-functional initiatives. Let us help you build yours.

WHAT IS COMMUNITY COLLABORATION?

It started with SDOH.

The world has known for some time that social determinants of health have a huge impact on peoples' lives. Some estimates suggest that 86% of the factors that determine health outcomes originate outside hospitals and doctors offices.

Some of those barriers - like racism and income inequality - are systemic and institutional, and very hard to change.

But others can be changed. When healthcare and social service organizations team up, they can assess, understand, mitigate, and sometimes even eliminate the social factors that negatively impact health and improve one's quality of life. It's practical, it's constructive, and it's being done right now.

That's community collaboration. And it's changing the world.
